책소개
이책을 정리하면서정형외과 전문의가 되면서 가장 꿈꾸는 일은 훌륭한 외과의가 되는 것이다. 많은 선배와 간혹 대가로 일컫는 분들의 수술을 보고 또한 본인이 수술을 흉내내면서 가장 중요한 것은 사전에 철저한 준비가 있어야 한다는 점이다.환자에 대한 올바른 진단을 위한 점과 과연 수술밖에 다른 선택이 없는가 환자 입장에서 심사숙고가 이루어 져야 한다. 수술을 잘 하시는 분일수록 수술적응증이 좁게 정해진 것을 볼 수 있다. 또한 대가(大家)선생님의 수술을 보게 되면 기본 원칙에 충실함을 볼 수 있었다. 고관절의 김영롱 스승님, 견, 주관절의 Matsen, 이용걸 선생님 수술을 보면 교과서에 써진 대로 일정하게 수술이 이루어 짐을 볼 수 있었다. 교과서 밖의 선생님만의 독특한 방식도 일반적 원칙에 충실하면서 발전하신 것이다. 수술결과가 예상 밖으로 빨리 좋아질 때, 가끔은 수술 안하고도 결과가 좋아질 수 있지 않았나 고민이 들기도 한다. 최근 국내 최다 수술을 자랑하는 의사가 초기 관절염 무릎을 앞으로 관절염이 진행될 것이니 미리 인골관절치환술을 하면 결과도 좋다고 하면서 수술을 적극적으로 권유하고 가끔 무료시술로 언론에 선행의사로 소개될때 잘못 들었길 바랄 뿐이다.한 분야의 환자를 계속 수술할 수 있으면 그 부위의 수술기법이 익숙해지고 수술시간도 점점 줄일 수도 있겠지만, 다양한 부위의 다양한 환자를 볼 수 밖에 없는 대부분 의사일 경우 그때마다 수술기법 공부 및 그에 필요한 해부학을 다시 한번 확인하고 주요 구조물을 파악하여 수술 중 당황하는 일을 줄여 나가야 할 것이다.이 책은 각 부위의 기본적인 해부학 지식을 바탕으로 기본적인 수술적 접근을 소개함으로써 짧은 시간에 해당 수술을 준비 할 때 특히 도움이 될 수 있다고 본다.최근에 최소 절개나 관절경을 이용한 수술이 많이 쓰이고 있다. 이러한 수술이 가능해진 것은 고전적인 수술이 바탕이 되고 의료기 등 주변과학의 발달로 인한 도움이 커서 가능해지고 더욱더 널리 쓰일 것으로 보인다. 그러나 고전적방법의 수술적접근을 제대로 알지 못하고 이러한 시술만 습득하는 것은 절름발이 수술이 되기 쉽다. 다양한 접근에서 가장 현명한 방법을 선택하지 못하고 숲을 보지 못하고 나무만 보는 우를 범할 수도 있다. 해당부위의 큰 시야를 위해 고전적인 수술기법부터 관절경 수술까지 발전해 나가는게 해당부위를 올바로 수술할 수 있다고 본다.실패를 반복하지말고 발전되는 외과의로 되기위해서는 기본 원칙에 충실하여야 할 것이고, 이러한 책이 많은 도움이 될수 있다고 본다. 우리말로 옮기게 되어 낯선 용어와 매끄럽지 못한 것에 부끄럽게 생각하며 초벌번역에 도움주신 후배 의대생과 가본의학 관계자분들에게도 감사드립니다.마지막으로 못난 의사의 든든한 후원자인 아내 미경과 은솔, 강욱에게도 사랑한다고 말합니다.

머리말

1984년 초판이 출간된 이래 정형외과의 수술시야 확보를 위해 해부학적 접근을 다룬 본 저서는 정형외과와 외상학의 많은 훌륭한 외과의 및 초심자까지 모두에게 수술접근의 교과서로서 위치를 차지하고 있습니다. 정형외과의 및 외상의에게 실제 유익한 내용을 보여줘 왔습니다. 정형외과는 급속한 발전을 보이고 있습니다. 이전 저서에서는 가능한 손상된 골주위의 생물학적 복원력을 유지하기 위해 혈액공급을 잘 유지하고 연부조직손상의 최소화를 강조하였습니다. 이번에는 대퇴골, 경골과 상완골의 골수강내 고정을 위해 세가지 최소 절개를 통한 접근법을 소개하고 있습니다. 다시 한 번 수술시 항상 골에 부착된 연부조직을 가능한 보존해야 함을 강조하고 싶습니다. 단지 적당한 수술를 위해 꼭 필요한 정도만 시야확보를 하시기 바랍니다.
관절경분야에서 괄목할 만한 발전이 이루어지고 있고 슬관절만해도 많은 개방(관혈적)시술을 대신하고 있습니다. 유사하게 유사하게 견관절에서도 급속한 발전이 이루어지고 있습니다. 그러므로 이번 새롭게 이들 관절의 관절경적 접근을 소개하고 있습니다. 특히 슬관절에서 전내측,전외측 통로를 견관절에서는 전방과 후방 통로를 통해 관절의 검사가 이루어짐을 소개하여 최근에 널리 쓰이게 되고 있습니다.선진국에서는 관절경 시술이 많은 분야에서 개방적 시술을 대신하고 있지만 의사는 고전적인 시술도 알고 있어야 합니다. 관련인대손상을 동반한 개방손상이나 관절경시술여건이 안되있는 지역에서는 근원이 되는 해부학지식과 이에 기반한 접근이 이루어져야 합니다. 이번 3판에서는 비구 접근에는 변화된 내용으로 특히 복합적이고 심한 손상시 후방접근술을 소개하고 있습니다. 덧붙여 총천연색의 삽화를 통해 흥미유도와 생동감을 향상시키고자 하였습니다.
